Returning 4G for Home

I want to return my Unlimited 4G for Home device because I was not happy with the service. I calledbe 14 days or signing up in order to cancel my subscription. It has bee more than a week to get the email with instructions on how to send my device back. However, now that I have it and am trying to send it back, when I have the postal code, it is invalid. What is the correct address to send my return back to?
